Our pricing reflects our commitment to each of our clients, ensuring clarity, continuity, and compassion for each case we acquire while avoiding saturation.


If a client's needs escalate to crisis-level or EOL support during an active retainer period, fees will be adjusted fairly. Previously paid retainer amounts may be credited towards crisis services at Clarity Care Coordination's discretion.

Assessment

$500 

Our assessment is a comprehensive, one-on-one review designed to give families clarity, direction, and a complete care roadmap. We take the time to understand the elder’s health history, medications, care needs, safety concerns, living environment, and support system, while listening closely to family concerns, goals, and priorities.

Following the assessment, families receive a clear, actionable care plan that outlines short-term, mid-term, and long-term recommendations. This plan helps families understand what needs attention now, what to prepare for next, and how to plan ahead with confidence, reducing guesswork, stress, and costly missteps.

When appropriate, we connect families with trusted referral partners including medical providers, home care, facilities, legal, financial, and community resources. Families are not handed a list and left to figure it out alone. We help identify the right options and explain how they fit into the overall plan. For many families, this assessment becomes the foundation for all future care decisions.

Ongoing Retainer

$400-$1,000/mo

The retainer provides continuous care coordination and advocacy as needs evolve over time. We act as a consistent point of contact, helping manage communication between providers, services, facilities, and family members. This includes scheduling, follow-ups, problem-solving, and proactive planning to prevent issues from escalating.

Families benefit from knowing someone is actively overseeing care, tracking details, and addressing concerns as they arise, reducing stress, confusion, and the burden often placed on loved ones.

Crisis Support

$1,500/mo

Crisis support is designed for moments when families are forced to make urgent decisions without time, clarity, or a clear plan. This may include sudden hospitalization, rapid decline, unsafe living situations, or unexpected care decisions.

We step in quickly to assess what’s happening, explain options in plain language, and help families navigate time-sensitive decisions with confidence. Our role is to bring structure, calm, and informed guidance, reducing confusion, emotional overload, and the risk of costly or irreversible missteps during high-pressure situations.

Families gain immediate support, clear direction, and an experienced advocate during moments when the stakes are high and decisions matter most.

End-of-Life (EOL) Support

$1,500/mo

End-of-life support provides comprehensive care coordination and guidance before, during, and after death. We support individuals and their families through hospice coordination, comfort care planning, communication with providers, and decision-making, always centered on dignity, wishes, and emotional well being.

Our role extends beyond the moment of passing. We assist families with after-death coordination, including guidance around next steps, required notifications, paperwork, logistics, and referrals for funeral, memorial, and administrative needs. Families are supported through what to do, when to do it, and who to contact during a time when clarity is often hardest to find.

Throughout the process, we serve as a steady presence, helping reduce overwhelm, prevent confusion, and ensure nothing important is missed. This service allows families to focus on presence, connection, and honoring their loved one, rather than navigating complex systems alone.
